Welcome From The CISD Nurses
We hope you had a wonderful summer and are ready for a great 2007-2008 school year.
If your child is in need of immunizations we would appreciate your speed in obtaining these from your physician. Please request a copy and bring these to the secretary in the school office, or bring these by to the nurse's office at Childress Elementary. The nurses may also be contacted at 937-6313.
Medications that are to be administered at school must be accompanied by a parents written note. It is best if the parent will come by and visit with the nurses regarding medication and the need for it.
Please fill out your child's Emergency Care Form with necessary phone contacts. In the case of an illness or emergency we will do our best to contact you if phone numbers of parent, family, or friends are available.
We are looking forward to a great year with your children.
