Student record requests may be done by completing the Student Records Request Form below:

If you are unable to fill out the Student Records Request Form you may send a written request to the address below with the following information:

  • Name as it appears on school records
  • Date of birth
  • Year of graduation or year last attended
  • Return address
  • Phone number where you may be contacted

There is a 7 to 10 day turn around for record request done by mail. Walk in requests are done Monday through Thursday between 8:30 am and 4:30 pm and on Fridays from 8:30 am to 4:00 pm.

If you are making a request for student records for someone other than yourself, you must present a signed letter of consent for release in order to obtain their records.
